Long term care in the chronic rheumatic diseases-transition of care between the adolescence and adult care

ABSTRACT
Some of the patients diagnosed in the childhood and adolescence continue to suffer from rheumatic diseases in the adulthood. Transition of the patient from pediatric care to the specialized rheumatology adult care is a very complicated process. Medical, psychosocial, educational and occupational aspects of this process as well as health care system organization realities should be taken into consideration. The aim of this publication is to present the necessity of building up the system of patients’ transition between the systems.
