ABSTRACT
Adolescents and young patients may have ongoing problems with active rheumatic diseases, particularly with juvenile idiopathic arthritis. They require continuation of care and transfer from pediatric to adult services. The process of “transition” ought to include collaboration between pediatric and adult healthcare providers as well as involvement the young people themselves. The exact documentation is crucial for transitional care.
