Heterotopic ossifications after two-stage septic revision knee arthroplasty in a rheumatoid arthritis patient

ABSTRACT
We present a case of a 68-year-old woman suffering from rheumatoid arthritis (RA). Due to infection of a knee prosthesis, two-stage treatment was performed: cement spacer, and revision arthroplasty. Staphylococcus aureus was identified from the operative samples. Radiograms carried out 3 months after the spacer implantation showed ossifications on the anterior cortex of the femur. Revision arthroplasty was performed after normalization of CRP, IL-6 and ESR. Ossifications caused significant limitation of ROM of the knee. It is the first case of an RA patient who presented massive heterotopic ossifications after an infection of a total knee arthroplasty (TKA) in our clinical practice. Revision arthroplasty of the knee, particularly of an S. aureus infection, caused a higher risk of heterotopic ossifications.
