This is the case of a 43 years old female patient with known medical history of systemic lupus erythymatosus (SLE). Following extensive evaluation with blood tests for immunological, viral culprits, cardiac ultrasound, chest and abdominal CT, heart MRI and left heart catheterization the diagnosis of cardiomyopathy and wet constrictive pericarditis secondary to her SLE was made. Treatment with b-blockers, ACE inhibitors, diuretics and corticosteroids was given with complete recovery of heart failure. SLE patients often manifest cardiac complications like cardiomyopathy, pericarditis, and the practicing physician should always bear in mind this possibility when treating such patients with relevant symptoms.
