The fate of rheumatoid arthritis patients with total hip and knee arthroplasty

Aim of the study: The objective of this study was to trace the fate of rheumatoid arthritis (RA) patients after total hip and knee arthroplasty.
Material and methods: The questionnaire study involved a group of 100 rheumatoid arthritis patients, with hip and knee replacement. To conduct the study a self-developed questionnaire and the HAQ-DI questionnaire were used. The study involved 57 patients, including 48 women and 9 men. Patients underwent one or two hip arthroplasty and/or one or two knee replacement procedures.
Results: It was found that 70% of patients rated the effect of each performed operation as very good or good and 6.7% as bad. Subjective assessment of overall health status was not compatible with the subjective postoperative assessment (Fig. 1–3). Only 15.8% of patients identified their health as good, 33.3% as poor. Overall, 75.4% of respondents felt that compared to the time before the operations their overall health was better. HAQ-DI results showed the improvement of quality of life in 46% of patients and no change in 12% of patients.
Conclusions: It might be concluded that the remote study in RA patients evaluating the impact of performed surgical procedures on patients’ quality of life is difficult due to the steady progress of the underlying disease. Assessment of the impact of the surgical treatments on various domains of quality of life of patients depends on the expectations of patients. The longer duration of disease (RA) before surgery is, the greater is satisfaction with joint replacement operations in lower limbs. These operations primarily reduced pain and decreased physical limitations, allowing better walking and independence in daily activities (getting up), despite progression of the underlying disease. Conducted operations contributed to increased social activity of patients, and through release of pain, improved mental status and reduced restrictions on their choice of work (Table I, II). RA patients even after undergoing a number of lower limb arthroplasty operations still remain optimistic about the next operation, including revision.
