Reumatoidalne zapalenie stawów jest najczęściej występującą przewlekłą chorobą zapalną o nieznanej etiologii. Mimo ustalonych kryteriów diagnostycznych, rozpoznanie tej jednostki nastręcza duże problemy, zwłaszcza we wczesnej fazie choroby. Trwają więc poszukiwania przeciwciał markerowych o dobrze ustalonej wartości diagnostycznej. Do niedawna jedynym laboratoryjnym kryterium diagnostycznym był tzw. klasyczny czynnik reumatoidalny klasy IgM, przeciwciało o bardzo dobrze poznanym i udokumentowanym udziale w patomechanizmie choroby. Od 2010 r. dodatkowym parametrem laboratoryjnym, który obok wysokiej swoistości i specyficzności ma także duże znaczenie prognostyczne, są przeciwciała dla cytrulinowanych białek ACPA. Głównym atutem tych przeciwciał jest to, że mogą one występować na wiele lat wcześniej przed pojawieniem się pierwszych objawów choroby. Zarówno czynnik reumatoidalny, jak i przeciwciała ACPA były rutynowo oznaczane w Instytucie Reumatologii niemalże od momentu pojawienia się wzmianek literaturowych na ich temat. Podejmowano także próby przeprowadzania własnych badań i wdrażania ich wyników do diagnostyki chorób tkanki łącznej. W artykule podjęto próbę prześledzenia historii tych badań.
